For years, a 32-year-old woman lived with severe menstrual pain and chronic pelvic pain. As her symptoms worsened, she sought care at multiple hospitals, yet her treatment remained limited to pain medication.
Over the previous six months, her condition had deteriorated significantly. By the time she reached specialist care, the disease had progressed deep into the pelvis. She was diagnosed with deep infiltrating endometriosis (DIE), together with a left ovarian endometrioma and multiple uterine fibroids — conditions that had significantly affected her quality of life and well-being.
Complex Disease. Complex Surgery.
On May 21, 2026, Prof. Adel Shervin performed complex laparoscopic surgery for the patient.
The procedure included excision of bilateral deep pelvic endometriosis, with left-sided disease involving the pararectal area; removal of the left ovarian endometrioma; and myomectomy for six uterine fibroids.
2026年5月21日， Adel Shervin教授为一名32岁深部浸润型子宫内膜异位症（Deep Infiltrating Endometriosis，DIE）合并多发性子宫肌瘤患者成功实施了一期根治性手术。
患者长期饱受严重痛经及慢性盆腔疼痛等症状困扰，近半年来病情明显加重。此前，她曾辗转多家医院就诊，仅被建议口服止痛药。随着疾病不断进展，病变已累及盆腔深部组织，同时合并多发性子宫肌瘤及左侧卵巢子宫内膜异位囊肿（巧克力囊肿），严重影响了其生活质量和身心健康。
本次手术成功实施了腹腔镜下双侧盆腔深部浸润型子宫内膜异位症（DIE）切除术（左侧病灶已侵犯直肠旁）、左侧卵巢子宫内膜异位囊肿剥除术及多发性（6个）子宫肌瘤切除术。由于病变范围广泛、解剖结构复杂，手术难度较高，对术者的解剖认知、微创技术及团队协作能力均提出了极高要求。

手术过程平稳顺利，术中及术后均未发生任何并发症，
该病例再次提示，子宫内膜异位症尤其是深部浸润型子宫内膜异位症（DIE）是一种容易被忽视和延误诊断的疾病。提高公众及医务工作者对子宫内膜异位症的认识，推动早发现、早诊断、早治疗，对于改善患者预后和生活质量具有重要意义。

Because of the extent of disease and the complexity of the pelvic anatomy, the procedure required advanced anatomical knowledge, minimally invasive surgical expertise, and close teamwork.
The surgery was completed successfully, with no intraoperative or postoperative complications reported.
Beyond One Surgery
This case represents more than a successful complex procedure.
It reflects a broader challenge faced by women living with endometriosis: symptoms may be overlooked or inadequately addressed while disease continues to progress.
Deep infiltrating endometriosis can be particularly complex. Greater awareness among both the public and healthcare professionals, together with earlier recognition, appropriate diagnosis, and timely access to experienced care, can make a meaningful difference in the lives of patients.
